Process of issuing a certificate of right to use a “Master“academic title

An applicant will deliver a brief application with a certified copy of a diploma to the address of CU Rector´s Office in Prague, Ovocny trh 5, Praha 1. There is an administrative charge of 3oo CZK for issuing this certificate. The applicant will pay the sum through a giro transfer form which will be sent to him/her after the application has been received. After paying the administrative charge a certificate is issued and will be delivered to the applicant by postal service as a registered letter.      

Application for Issuing a Certificate of:

• academic title recognition (title before a name)

• change of abbreviation from “Dr.“ to “Ph.D.“ and “Th.D.“ (after a name)

• conferring a “doctor´s“ title in brief form “Ph.D.“ on the graduates from doctoral study on faculties of medicine (after a name)

(Concerning the graduates who finished their doctoral study until 31^st December 1998)

(§ 43 of the Czech Law on Universities No 172 Coll. on Higher education Institutions)

must be supplied with

• a brief application

• a copy of doctor´s diploma

• a copy of master´s diploma (if the applicant applies for a certificate of recognition of academic title before a name)

An amount of 370 CZK required by CU in Prague for issuing one certificate will be payed by a giro transfer form which will be delivered from a rector´s office to the applicant on the basis of the application.

The certificate will be sent to the applicant in 30 days.
