            * Process of issuing a certificate of right to use a “Master“academic title

            An applicant will deliver a brief application with a certified copy of a diploma to the ad
            Rector´s Office in Prague, Ovocny trh 5, Praha 1. There is an administrative charge of 3oo

            this certificate. The applicant will pay the sum through a giro transfer form which will b
            after the application has been received. After paying the administrative charge a certific

            will be delivered to the applicant by postal service as a registered letter.

            * Application for Issuing a Certificate of:
            *=========================================================================================

            • academic title recognition (title before a name)
            • change of abbreviation from “Dr.“ to “Ph.D.“ and “Th.D.“ (after a name)
            • conferring a “doctor´s“ title in brief form “Ph.D.“ on the graduates from doctoral study

              medicine (after a name)
            (Concerning the graduates who finished their doctoral study until 31st December 1998)
            (§ 43 of the Czech Law on Universities No 172 Coll. on Higher education Institutions)


            must be supplied with
            • a brief application

            • a copy of doctor´s diploma
            • a copy of master´s diploma (if the applicant applies for a certificate of recognition of
              before a name)

            will be addressed to

            An amount of 370 CZK required by CU in Prague for issuing one certificate will be payed by
            form which will be delivered from a rector´s office to the applicant on the basis of the a
